It is also known as the fishbone diagram or Ishikawa diagram and developed by Kaoru Ishikawa. It is useful in determining the root cause problems. It helps us to analyse all the possible causes of the problem, rather than just the ones that are most obvious and known.
A possible root cause can be uncovered using why-why and how-how diagrams as in Figures 8.17 and Figure 8.18
Causes in a cause and effect diagram (Figure 8.19) are frequently arranged into five major categories like manpower, methods, materials, measurement and machinery. We can also add more classifications as per the situation which suits our need.
