The Secretary of State’s website offers access to required forms for nonprofit organizations at: http://www.sec.state.ma.us/cor/corpweb/cornp/npfrm.htm. The filing fee is $35 and should be made payable to the Commonwealth of Massachusetts.

The Commonwealth requires charitable organizations to file annual reports by November 1 of each year. The filing fee is $15.
