1. | In the Fill Out a Form task pane, click Design a Form, and then click Customize a sample. |
2. | Click the form you want, and then click OK. |
3. | In the task pane, click the aspect of the form you want to modify. |

6. | When prompted to save the file, click Yes. |
7. | Browse to the location where you want to save the form, in the File name box, type a name, and click Save. |
