What follows are various form letters that may be adapted to your own specific situation and used accordingly. It is prudent to send these letters by certified mail, return receipt requested, in order to have a record of exactly when your letter was both sent and received. Also included is a form developed by the FTC to guide and record your efforts to correct identity theft problems.
