Red eye is a common problem in flash photos, especially photos of children or photos taken in dim light. When your subject is staring straight at the camera and you take a flash photo, the light from the flash might bounce off the retina at the rear of the eyeball and be reflected back to the camera. Because the retina is filled with blood vessels, the reflection is red—hence, red eye.
